Advanced Clinical Practitioner

As an Advanced Clinical Practitioner (ACP) with North Hampshire Urgent Care (NHUC), you will provide highquality autonomous clinical care to patients with undifferentiated and urgent primary care needs. Working across telephone triage, face‑to‑face consultations and home visits, you’ll assess, diagnose, treat and refer patients within your professional scope, supported by robust clinical pathways and a collaborative multidisciplinary team.

You will use your advanced skills across the four pillars of practice to deliver safe, effective care, contribute to service development, support colleagues, and maintain the highest standards of professional practice.
