Collaborating with clients, families, and healthcare partners, you'll promote resilience and empowerment through effective care strategies and evidence-based practices.

Key Responsibilities:
• Lead the creation of Individual Care and Health Action Plans
• Offer services in accessible formats, including telehealth
• Connect clients with needed social supports and resources
• Advocate for clients’ healthcare needs and preferences
• Conduct outreach for engagement in ECM services

Requirements:
• Bachelor's degree in an appropriate field required
• Minimum three years of relevant experience
