Clinical Pharmacist – Framlingham Surgery

Framlingham Medical Practice is looking to recruit a Clinical Pharmacist to work 25-30 hours per week (flexible). The role is based in Framlingham, Suffolk and requires excellent interpersonal and communication skills, strong time‑management, and the ability to work effectively as part of a multidisciplinary team.

Responsibilities

• Reduce the medicines‑related workload on doctors.
• Increase safety and quality of prescribed medicines.
• Provide support on medicines‑related issues to the whole practice team.
• Be a point of contact for patients regarding medicines queries.
• Conduct clinical medication reviews.
• Manage chronic disease medication (e.g., hypertension, type 2 diabetes, asthma, COPD, polypharmacy reviews).
• Perform medicines reconciliation on transfer between care providers.
• Conduct care home/frailty medication reviews.
• Focus on QOF domains, improving disease registers.
