Community Living Facilitator Role at CLH

Enhance the lives of individuals with diverse abilities at Community Living Hamilton as a Direct Support Facilitator. Support transitional aged youth in achieving independence and skill-building every day.

As a Direct Support Facilitator, you will assist clients in our Enhanced Supported Independent Living program at CLH. Your responsibilities include developing personalized support plans and advocating for clients’ needs with community partners. Previous experience with developmental disabilities, mental health, and addictions will greatly benefit your role.

Key Responsibilities:
• Assist transitional aged youth with skill development
• Advocate for clients’ interests in the community
• Develop Individual Support Plans addressing personal goals
• Ensure safety and wellness for all clients
• Build effective professional relationships grounded in respect

Requirements:
• DSW, SSW, CYW, or related degree/diploma
