Description
Provide support and advocacy to maintain family unit. Participate in all case conferences, and planning for all children and families Case notes all meetings and provide update in client files Monitor status and implementation of all open files with all Child Welfare agencies Follow up with families to ensure all information is correct and services are being provided Make referrals to appropriate resources Provide transportation to ensure children are kept out of care, should protection not be warranted. Liaison with community resources to provide support to families.
