Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Writing, training and monitoring the Staff Action Plans, Safety Support Plans, Personal Expenditure Plans, and Money Management Assessments.
• Assures any plans that address safety needs are accurate and complete, and safeguards are in place.
• Participate in, and facilitate person-centered Life Plan Meetings for those on assigned caseload. Within the interdisciplinary team, acts as a liaison for the individual, while advocating for appropriate goals, supports, and services.
• Coordinates the scheduling of special team meetings, which may include the participation of outside agencies or entities.
• Helps to coordinate and participate in service delivery, recreation, and ensure program supply needs.
• Conducts new employee training, and on-going refreshers related to EHR and documentation.
• Will attend and participate in all mandatory trainings and staff meetings.
