Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.

The Motor Neurone Disease (MND) Share Care Worker is a regional role in the Hume Region of Victoria and an integral part of the Palliative Care team, whose purpose is to raise the quality of life for people living with MND by:

• Promoting early referral to palliative care services
• Support, development and delivery of education to palliative care services providers
• Provide coordination between palliative care services, MND clients and families in the Hume region.
