An organization in partnership with Nova Scotia Health is seeking four (4) full‑time Nurse Practitioners (NPs) to provide interim coverage from April through June 2026 , supporting a Transitional Care Facility prior to permanent NP hires. This is a unique opportunity to practice in an innovative, restorative model of care, within a collaborative inpatient environment.
About the Transitional Care Facility
- The facility supports adult patients (18+) in Nova Scotia’s Central Zone who: No longer require acute care (medically ALC with non‑medical discharge barriers).
- Require additional time, services, and support before safely returning to the community.
- Benefit from a restorative, activating environment focused on regaining and maintaining functional independence.
