Job Description

The Mount Sinai Academic Family Health Team – Vaughan site is looking for a Nurse Practitioner to join our interprofessional primary care team. As an integral member of the team, you will provide safe, high-quality, patient-centred care to a diverse patient population and work collaboratively with the interprofessional team to support comprehensive healthcare delivery within an academic family health setting.

In this role, you will provide comprehensive care across the lifespan, managing both acute and chronic health conditions while promoting wellness, prevention, and health education. You will also play a key role in supporting education and professional development across the team, including formal preceptorship of Nurse Practitioner students and supporting the clinical learning of other learners, such as residents and clerks.
