This role is ideal for a dynamic healthcare professional with strong clinical reasoning skills and a passion for discharge planning, supporting patients in achieving optimal recovery and successful reintegration into the community.
The successful candidate will collaborate closely with allied health and other members of the interprofessional team to deliver exceptional, patient-centered care.
