Seasons Care is looking for passionate Registered Dietitians dedicated to enhancing the health of seniors in Long-Term Care. You will identify nutrition risks, develop individualized care plans, and provide education to residents and families. Your role will include conducting assessments and collaborating with multidisciplinary teams to implement dietary interventions.
Key Responsibilities:
• Monitor nutrition and hydration levels for all residents
• Complete RAI assessments upon admission and quarterly
• Develop and modify individualized nutrition care plans
• Liaise with residents and families for effective interventions
• Collaborate on enhancing the dining experience
