Description
The Registered Nurse (RN) - Assertive Community Treatment Team (ACTT) plays a critical role in delivering comprehensive, community-based mental health care to individuals with severe and persistent mental illnesses. This position focuses on providing holistic nursing assessments, medication management, and therapeutic interventions within clients' homes and community settings to promote recovery and improve quality of life. The RN collaborates closely with multidisciplinary team members to develop and implement individualized care plans that address both physical and mental health needs. By engaging clients in their own care and supporting their social integration, the RN helps reduce hospitalizations and enhances community functioning.
