Registered Nurse Care Coordinator—Interprofessional Pain & Addiction Recovery Clinic, Centre for Addiction and Mental Health (CAMH).

Full-time, temporary position (until August 27, 2027) located at 100 Stokes Street.

Responsibilities

• Participate in motivational interviewing and client assessments.
• Liaise with the care team to provide pharmacological and non‑pharmacological treatments in pain and addiction management.
• Perform case management duties.
• Collaborate with internal and external stakeholders.
• Support a diverse, inclusive workplace and comply with applicable standards and requirements.

Qualifications

• Bachelor of Science in Nursing (BScN) and registration in good standing with the College of Nurses of Ontario (CNO).
