As a member of an interdisciplinary team, the Registered Practical Nurse (RPN) provides patient-centered nursing care within the Way Home Unit, supporting patients transitioning from hospital to home. The RPN may be redeployed to other patient care areas as organizational needs arise and may also provide care in the community through the OSMH at Home program when assigned.
