Overview

Senior Manager, Integrated Care Programs is accountable for supporting and evolving Home Health Integrated Care Programs through development and fostering robust internal and external relationships. Acting as a liaison between external partners and internal departments, the role is vital to the sustainability and growth of existing programs and to the implementation and oversight of new initiatives.

Responsibilities

• Act as the primary point of contact for community partners (funders, vendors and subcontractors) for integrated care programs.
• Participate in program operational co‑design working groups with internal and external stakeholders.
• Maintain a solid understanding of stakeholder needs and gaps (potential opportunities) with integrated care programs to ensure CBI Home Health is positioned to respond with innovative solutions.
