Our Transitional Case Management Program embodies this belief by supporting individuals experiencing addictions and mental health needs to strive for improved quality of life. Transitional Case Managers (TCMs) support the transition from hospital-based care (LHSC) to community services which is often a very critical time in a person’s health journey. TCMs develop strong working relationships and collaborative partnerships with other providers including but not limited to LHSC, addiction services, and social supports.
