POSITION TYPE PERMANENT, FULL-TIME
OVERVIEW: Reporting to the Memengwaa Supervisor, the Transitional Youth Worker provides case management supports to youth on an extension of care who are living in or transitioning out of Memengwaa transitional housing units. The key objective of this role is to support youth to build their personal capacity, in preparation for the next stages of their lives.

KEY RESPONSIBILITES:
Provide case management services to Indigenous youth in and from care.

Alongside team staff, screen referrals and complete intakes to the program.

Utilizing program tools, complete SMART goal setting and life skill assessments with tenants; follow up on goals and assessments ongoing.

Conduct weekly check-ins with youth to implement, review and update goals.

Refer to and liaise with appropriate resources; advocate for youth when required.
